One Community Health Data Breach

On April 19 and 20, 2021, One Community Health experienced a data breach where an unauthorized third party gained access to its servers where sensitive patient and employee data was stored. One Community Health waited until November 22, 2021 to notify people that their information was accessed by unauthorized persons.
One Community Health is a private, non-profit community health center serving the greater Sacramento, California area. If you received a notice from One Community Health, you were impacted by the breach.
What Information is Involved?
According to One Community Health, the following information was affected:
- Social Security numbers
- Full names
- Dates of birth
- Addresses and other demographic information
- Health insurance information
- Diagnosis information
- Treatment information
This information is called your Personally Identifiable Information (“PII”) or Protected Health Information (“PHI”). It tells others about you and is considered part of your identity. Businesses are required to secure this information or risk facing statutory penalties, among other legal penalties. Stolen PII and PHI can be used by identity thieves to engage in fraudulent activity using your identity.
One Community Health is offering one year of free identity protection services. The best way to protect yourself after a data breach is to sign up for identity protection services as soon as possible. California offers extra protections and legal rights to its residents through the California Consumer Privacy Act.
