According to the notice, the breach was identified following unusual network activity detected on June 10, 2025, which prompted an immediate internal investigation. On July 9, 2025, the investigation was completed, and PIIC began to mail notification letters to impacted individuals on July 22, 2025. These letters include an offer of 12 months complimentary credit monitoring, fraud consultation, and identity theft restoration through Kroll.
